Other referrals will come from: NHS, Social Services (Enablement team), Private Hospitals, GP Surgeries, Hospices, Private individuals/families and Care Agencies

Palliative Care: We deliver palliative care in support of both the family and patients with severe illness. Our therapeutic service enhances the quality of life of those affected by the trauma caused by the serious medical condition.

Domiciliary Services: We provide domiciliary care for needs resulting from physical frailty, continence mental health support, disability, learning difficulties. We can offer services to help carry out household chores, physically demanding activities, shopping, home cooking and laundry. We can also make arrangements for live-in carer support.

Continuing Care Services: We also cater for those experiencing long-term conditions or degenerative ailments and therefore are not able to cope independently. Our professional nurses attend to the needs of bedbound patients, those requiring bowel care, catheter care and tracheotomy care

Personal care includes:
• Bathing
• Dressing
• Assisting with medication
• Assistance with meals

Care: All of our care is delivered by highly trained members of the team and implemented with a set of robust monitoring and review process which allows us to maintain the highest standards of care. This also ensures that we tailor our services to meet the ever-changing needs and choices of each service user.

Central to our service of care is our care plan document, which allows us to monitor the evolving needs of the individual. We endeavour to involve both the clients and family in the creation and management of the person-centred care plan This person-centred approach emphasises respect, dignity and quality of life.

We understand that needs are ever-changing therefore the care plan is reviewed every six month by key personnel to ensure that needs are being consistently met.

To continuously improve our service, we also undertake regular performance and quality assurance monitoring which measures our services against defined industry benchmarks.
